IN31T1C - PROFESSIONAL TRAINING IN COMMUNITY AND TERRITORY

The internship, in compliance with the provisions of the Teaching Regulations, Article No. 3 "Specific training objectives and expected skills," aims to provide, under the supervision of a clinical tutor, knowledge of preventive and care interventions designed to address priority health problems in outpatient, community, and home settings, during the acute phase of the disease and as educational and psychosocial support during the rehabilitation and chronic phases.


EXPECTED LEARNING OUTCOMES
KNOWLEDGE AND UNDERSTANDING
- identifies behavioral measures to prevent occupational risks for the safety of oneself and others;
- identifies the health and nursing care needs of individuals, families, and the community, identifying priority needs, and formulates related care plans;
- plans the main interventions related to clinical/care conditions, knowing how to formulate Nursing Diagnoses and identify collaborative problems, evaluating the outcome;
- promotes effective educational relationships, aimed at preventing illnesses and maintaining and recovering satisfactory health;
- develops the skills necessary for effective teamwork.


ABILITY TO APPLY KNOWLEDGE AND UNDERSTANDING
- applies the principles necessary to protect their own and others' safety (staff and patients);
- adopts occupational risk prevention strategies;
- ensures correct and effective procedures in application and compliance with the fundamental scientific principles of the profession and adapts services to the patient, in relation to different pathologies/clinical conditions, different care settings, and different age groups;
- plans and delivers nursing care in collaboration with patients and the multidisciplinary team;
- initiates and maintains care relationships aimed at health and therapeutic education with the patient and their family, adopting effective communication and adapting language to the level of understanding of the interlocutors;
- plans, intervenes, and evaluates the care provided in compliance with guidelines, protocols, and procedures based on the best available scientific evidence.
- activates and involves the patient's social and emotional network in the care plan, including through therapeutic education and rehabilitation programs, to develop self-care skills and problem management.
- plans, intervenes, and evaluates the care provided to chronically ill, frail, and disabled patients in the community.
- documents the planned, delivered, and evaluated nursing care under the supervision of the clinical tutor.
